To My Patients,
After 40 years of practicing medicine in Phoenix, I've been working on plans to retire for some time.
Given the current COVID-19 healthcare emergency and the need to quickly transition clinics to telemedicine, I have made the decision to expedite my impending retirement.
It's with great confidence that I refer you to Daniel Charous, M.D. and his team at Arizona Desert ENT. Dr. Charous has a long-standing history in the valley and looks forward to welcoming you into his practice and providing you with excellent care. Dr. Charous' office is equipped with the latest technology available, including telemedicine.
Dr. Charous and I will be working together closely to provide continuity of care. We both want to make this transition as easy as possible for you, our patient.
Patients should rest easy knowing that clinical records are stored securely in a third party medical records management facility at the direction of the custodian of record.
In accordance with HIPAA, patients of Dr. Baldree are welcome to be seen by Dr. Charous or have their records transferred to a different physician, or obtain a copy of their file. To do this, please contact the staff at Arizona Desert ENT, and they will have the custodian of record make these arrangements for you.
